2. Letters of reference OR name, address, organization, titles and phones numbers of two referees.
3. Letter from sponsoring Organization/Agency.
4. Application fee of $35.00 (Non-refundable).
   
   

NB: Enrolment fee ($100.00) is not due until acceptance is given to the applicant, in writing.
